We invite you to join us in creating a collaborative environment for the health care community and health services researchers to share and discover how telehealth technologies are being used in practice settings. 

The vision for the Innovation Exchange is to create a space where providers can find technology-enabled models of care that they can trust. We are excited to engage with you and to hear about your innovative approaches to preventing, diagnosing and managing a variety of health conditions. We are currently seeking telehealth-enabled models of care for diabetes, cardiovascular disease and related conditions.
